    Nucleotomía percutánea automatizada: experiencia en 425 casos

    Our experience using automated percutaneous nucleotomy for treatment for herniated disc is presented. A total of 42 5 patientes, aged between 18 and 58 years and operated from June 88 to December 1992, has been included. Patients were clinically assessed at 6 weeks, 3 months and 6 months after surgery. Satisfactory results were found in 71% of cases. As for complications, there was only a case of discitis. In 29% of patients, the outcome was poor. An important group of failures were due to bad selection of patients regadings psychological profile.

    Estudio histomorfométrico del hueso diafísario en la rata

    A histomophometric study of the femoral diaphyseal bone was undertaken in «Wistar» albine rat. Measurements of the endosteal and periosteal thickness as weel as variations in cortical, medular and total bone areas in relation to the endosteal and periosteal bone activity during the first year of life were analyzed. The results showed a continuous growth increase with a phase of lower growth rate from the 25th to the 45th day of life. The pattern of diaphyseal growing changes resembled that found in humans. The rat can therefore be considered as a suitable experimental animal for the study of bone remodeling during growth.

    Evolución de las curvas sagitales en pacientes afectos de escoliosis tratados con corsé de milwaukee

    A retrospective analysis was carried out in a group of 41 patients affected by idiopathic scoliosis treated by Milwaukee brace. The aim of the study was to asses the effect of the brace in the evolution of the thoracic sagital curve. We observed a higher frecuency of cases with thoracic lordosis in patients with less than 12 years of age, or in those treated for more than 3 years. There were cases who wore the brace for more than 5 years with no modification in the sagital curve. In conclusion, the Milwaukee brace induced a lordotic effect in just one third of patients treated for idiopathic scoliosis. The onset of treatment in patients under 12 years and long a periods of treatment should be considered as the predisponing factors for thoracic lordosis.

    Tratamiento quirúrgico de la luxación recidivante de rótula mediante técnica combinada de realineación proximal y distal

    Los autores presentan su experiencia en el tratamiemto quirúrgico de la luxación recidivante de rótula, en una serie de 26 pacientes esqueléticamente maduros (27 rodillas), intervenidos tras fracaso de un periodo mínimo de seis meses de tratamiento conservador. El tiempo medio de seguimiento postoperatorio fue de 20 meses. La técnica quirúrgica realizada fue la realineación proximal según Insall, asociada a realineación distal tipo Emslie-Trillat. La patología intraarticular concomitante se resolvió mediante artroscopia previa en el mismo tiempo quirúrgico. Las complicaciones encontradas fueron, un caso de hematoma postoperatorio, dos cicatrices queloides, y cuatro casos de rigidez articular. Los resultados fueron excelentes en 21 pacientes. En cinco casos se objetivó clínica de sufrimiento patelar, postquirúrgico. Los buenos resultados obtenidos parecen tener relación con la seguridad que proporciona la técnica de realineación combinada proximal y distal unida a un programa meticuloso de rehabilitación precoz.

    Tratamiento de la luxación congénita de cadera inveterada según la técnica de klisic

    A group of 15 patients, over 6 years of age, with neglected congenital hip dislocation (16 hips) were operated on using the Klisic''''s technique. The mean follow-up was 4 years. Prior this surgery, 12 hips had undergone different surgical treatments without satisfactory results. At the end of follow-up 11 hips were satisfactory reduced. In 6 cases necrosis of the femoral head was detected, but necrotic signs were seem in the preoperative radiographs of 5 of these cases. According to a clinico-radiographic score (Trevor et al. 1975), either excellent or good results were obtained in 50% of cases. In 3 1% the outcome was poor from both clinical and radiographic point of view. In conclusion, Klisic''''s technique seems to be a suitable alternative for the treatment of reglected congenital dislocation of the hip. The more proper indication would be a high dislocation of the hip with hypoplasic acetabulum.
