In conclusion, although still in its infancy, a hopeful field for the use of minimally invasive surgery in ESCC beyond the LACC trial is emerging. Nevertheless, further evidence based on randomized, prospective, multicentric clinical trials, like the CONTESSA/NEOCON-F protocol (5), is required. Indication for FST in ESCC should encompass an accurate selection of patients, based on oncopathological findings and a whole evaluation of the reproductive context of the patient. All in all, our recommendation should lie on the ethical principle that the management of patients with cervical cancer, in relation to the preservation of fertility, always should be considered without a concomitant sacrifice of the oncologic outcomes
