    Estrés oxidativo y osteonecrosis de la cabeza femoral

    Non-traumatic osteonecrosis of the femoral head represents a significant clinical problem. Chondral collapse occurs in up to 80% of the untreated cases resulting in bone destruction, pain and loss of joint function and prosthetic hip replacement at an early age is needed. The pathogenesis of the disease is likely multifactorial and seve- ral etiologic factors are known to be associated with it: alcohol intake, smoking, corticosteroid use, impaired hemos- tasis and lipid metabolism, and the presence of certain polymorphisms. However, a significant number are described, as idiopathic and do not present any risk factor. Recent studies in steroid-induced ONF animal models suggest that oxidative stress could be involved in the development of the disease. These studies show that the administration of pro-oxidant substances increase the risk of disease and antioxidants administration might prevent the development thereof

    Profesionalismo

    El profesionalismo no es una estructura estática con la que podemos proporcionar asistencia médi- ca, sino que debe contemplarse como un sistema dinámico derivado de la interacción de muchas fuerzas. Este traba- jo se ocupa de definir el profesionalismo, conocer su evolución a través de generaciones y de su enseñanza a estu- diantes y profesores. La profesión debe adoptar nuevos valores para mejorar la perspectiva y eficacia de los médicos modernos en la sanidad actual. El profesionalismo es la principal fuerza para luchar contra el comercialismo en medi- cina. Finalmente se presenta la idea del profesionalismo en la práctica de la cirugía ortopédica en el nuevo milenio.

    Rotura aguda completa del ligamento colateral metacarpo-falángico radial del pulgar: una lesión grave infrecuente

    El ligamento colateral radial es el principal estabilizador articular contra las fuerzas de estrés en aducción de la articulación metacarpofalángica del pulgar. Esta lesión infrecuente, pasa a menudo desapercibida, desarrollándose una inestabilidad articular sintomática y, posteriormente, una artrosis secundaria. Aunque tradicionalmente se ha recomendado el tratamiento conservador de estas lesiones, estudios recientes han documentado mejores resultados funcionales cuando se realiza su reparación quirúrgica, especialmente en las lesiones completas.

    Tratamiento quirúrgico de fracturas tipo A3 de húmero proximal

    32 cases of A3 type fracture of the proximal humerus are presented. They were treated surgically by reduction and internal fixation with a proximal humeral nail or a locking plate. The results were satisfactory at most of the patients concerning the residual pain (mean Constant-Murley pain score: 11,88/15), the strength (mean Cons- tant-Murley strength score: 15,13/25) and joint mobility of the shoulder (mean Constant-Murley mobility score: 20,88/40). The most important complications were rotator cuff disorders (34,4%), varus consolidation of fracture (31,3%) and prominent hardware to the joint (12,5%). A3 type fractures of the proximal humerus are injuries that evolve normally well, when the surgical technique is performed correctly.

    Exéresis de la cabeza radial vs prótesis de sustitución en las fracturas complejas de la cabeza del radio

    The aim of this study was to describe our results in the treatment of Mason III radial head fractures and compare the outcomes of the radial head excision and prosthetic replacement. We performed a restropective study of radial head fractures treated in our departmet. Were included for the study 26 patients with Mason III radial head fractures. The follow-up time was 28 months. A total 14 patients (56%) were treated by excision of the radial head (Group A) and 12 patients (44%) were treated by the prothesis (Group B), The functional results according to the Bro- berg-Morrey scale were 4 patients excellent, very good 15 patients, 6 patients regular and 1 patient bad. When com- paring group A with group B there were no differences in age, sex, and postoperative functional outcomes, however, patients in group A had better functional outcomes when grouped excellent and good results (75%).
