    Avulsión del tubérculo tibial asociada a epifisiólisis proximal de tibia

    Fractures of the anterior tibial tubercle are uncommon lesions, adolescents being more frequently affected. Physeal extension through the posterior tibial cortical bone is rare. Some authors have suggested that a new type IV can be added to the Watson-Jones classiflcation. In this paper, we report a case with epiphysiolisis of the proximal tibia associated to an avulsion fracture of the anterior tibial tubercle. The Watson-Jones classiflcation, the mechanism of injury are discussed together a review of the literature.

    Metalosis en prótesis total de cadera modelo PM A propósito de 1 caso : a propósito de 1 caso

    We report a case of a patient who had aseptic loosening of a total hip prosthesis model PM. At revision we found severe metallosis. A black surgery, seven years after the device was implanted pigment that penetrated the capsule and sorrounded the femoral and acetabular components was found. Histological examination of a the specimen of capsular tissue showed histiocytes loaded with small size metallic particles. Of particular interest was the absence of severe clinical and radiological changes. These finds are unusually associated with aseptic loosening níade by polymeric wear debris. The reason of metallosis was supposed tobe the friction between the neck of femoral component and lower rim of the acetabular component.

    Los corticoides epidurales en el tratamiento del síndrome del canal lumbar estrecho

    The authors present a study on 67 patients with degenerative lumbar spinal stenosis. The treatment consisted in epidural injections with corticosteroids and a program of physical therapy. There was 80% of good results. A clear influence of the physical therapy program on the quality of the final result, has been proved.

    Mielopatía compresiva por occipitalización del atlas

    Se presenta un caso clínico de occipitalización del atlas, alteración congénita caracterizada por la existencia de una fusión parcial o total del anillo óseo del atlas con la base del occipucio. En el 50% de los casos se asocia una impresión basilar que origina síntomas neuroló- gicos por compresión neural si la odontoides sobrepasa la abertura del foramen magnun. Es importante el determinar la existencia de esta anomalía, incluso ante síntomas comunes como dolor cervical y vértigo. La cirugía es la terapéutica de elección, principalmente cuando existe inestabilidad atloaxoidea, y consiste en la estabilización articular mediante fusión posterior cervical suboccipital asociada en casos de grave compromiso neurológico de resección transoral de la odontoides. En nuestro caso el paciente fue sometido a tratamiento conservador al rechazar voluntariamente la intervención.

    Utilización de fijadores externos circulares en el tratamiento de la pseudoartrosis infectada de tibia secuela de aplastamiento de miembros

    Se revisan 7 casos de pseudoartrosis infectada de tibia, secuela de aplastamiento de miembros, tratadas inicialmente en el momento de la fractura con desbridamiento y fijador externo monolateral. Todos los casos eran iniciamente fracturas abiertas grado III de Gustilo. El tratamiento de la pseudoartrosis se realizó mediante desbridamiento, estabilización con fijador externo circular, cobertura de partes blandas e injerto óseo. La unión ósea se consiguió en todos los casos con una buena recuperación funcional. Se discuten las ventajas del fijador externo circular en estos casos.

    La prótesis de expansión de Giannini en el tratamiento del pie plano-laxo infantil

    The authors present their experience in the surgical treatment of 50 essencial valgus fíat feet, performed bctween 1989 and 1992, using Giannini's subtalar expansive prosthesis. Pacients were assessed clinically, radiologically and by photopodography for the Moreau-CostaBartani angle and the Sheade line or Meary-Tomeno axis. In the final evaluation 14 cases (28%) were judged to achieved excellent results, 31 (62%) good results, 2 (4%) fair results and 3 (6%) poor results

    Resultados a largo plazo de la patelectomía total

    Nineteen total patelectomies were performed since 1981. The indication was femoropatellar osteoarthritis in nine cases, anterior knee pain syndrome in eight, and conminute fractures in two. The mean age of the group at the time of operation was 48 years (25-80). A1l the patients with anterior knee pain syndrome had been operated previously. With an average followup time of 7.8 years (3-14), vve reviewed our patients clinically and radiographically. We used the International Knee Society rating scale and found 58% of excellent and good results; 100% of the patients operated because of osteoarthritis and 50% of those operated because an anterior knee pain syndrome have pain at review. Total patellectomy is not a frequent technique. Today, the sole indications are very conminute fractures of the patella, femoropatellar osteoarthritis in those patients in which total knee arthroplasty is not indicated and in the anterior knee pain syndrome not improved with other treatments.

    Alteraciones anatómicas y funcionales en la muñeca después de la fractura de la extremidad distal del radio

    A series of 75 cases with fracture of the distal radius treated by conservative methods were retrospectively reciewed. Results were evaluated from anatomical and functional standpoints. In 49% of cases there was an articular extension of the fracture, and 23% had an associated fracture of the ulnar styloid process. Satisfactory outcome was achieved in 60% of cases, although there way 37% of cases showing complications during follow-up.

    Tratamiento de las fracturas complejas de fémur mediante enclavado intramedular encerrojado de Brooker-Wills

    A retrospective study of 19 patients with complex fermoral fractures treated with the Brooker-Wills interlocking intramedullary nail is reported. The traffic-accident was the most common cause of these fractures. Associated injuries were found in 63% of cases. The mean time follow-up period was 27 months. Results were assessed according to Thorensen's criteria. We found excellent or good results in 84% of these patients. There were intraoperative technical problems without clinical complications in 8 patients, most of them due to distal femoral cortex penetration with the pins. Union occurred in all fractures. The greatest advantage of this nail comes from the easy insertion of distal pins, providings less exposition to radiation and a shorter operative time
