- Lesiones óseas por tracción : a propósito de un caso(2017) Segura Sánchez, David; Mínguez Rey, María Fe; Pino Almero, L.Injuries of the pelvic apophyses due to traction, such as apophysitis and avulsion fractures, are not common. They are especially seen in adolescents and young adults who take part in sport activities. We present the clinical case of an athletic adolescent with apophysitis of the anterior inferior iliac spine whose clinical presentation and complementary tests simulated a bone tumor. An appropriate knowledge of this entity will allow suspecting it, diagnosing it early and can avoid confusing it with potentially malignant bone lesions. The prognosis of these injuries with conservative treatment is favourable in most cases.
- Lesión lítica en la epífisis de un paciente joven con anorexia : a propósito de un caso(2017) González Buesa, Eduardo.; Malillos Torán, ManuelEl condroblastoma es un tumor poco frecuente, benigno aunque localmente agresivo, que suele asentarse en la parte proximal de tibia o fémur y afectar a individuos entre 15 y 25 años de edad. Se presenta un caso clínico cuya principal manifestación es el dolor y que se acompaña de un síntoma de alarma, la anorexia.
- Corrección de deformidad postraumática del radio distal secundaria a puente óseo fisario(2017) Aguilella Fernández, Luis; Maruenda García Peñuela, B.; García Martínez, LucíaDistal radius fractures in the childhood are frequent and they are usually treated in a conservative fashion, although some cases need percutaneous pinning. A physeal bone bridge is an uncommon complication of K-wires insertion and it may provoke a severe deformity while growth potential remains. A 9-year-old boy with this complication is reported. He was successfully treated by physeal distraction, achieving a satisfactory correction of his deformity.
- Evolución y tratamiento de los pacientes con pseudoartrosis de tibia de etiología traumática(2017) Acebrón Fabregat, A.; Mifsut Miedes, Damian; Gomar Sancho, Francisco; Silvestre Muñoz, AntonioThe evolution and results of eight patients with traumatic tibial pseudoarthrosis has been examined. Four of them had openned fractures and the rest were closed. The most frequent cause was a running over (half of them). Surgery was the first treatment in all of them, being the endomedular locked nail the preferred osteosynthesis method. They all needed a second surgery, and some of them, even a third one. The average consolidation time was 27 months, and the results were excellent in most of them.
- Reconstrucción del ligamento cruzado anterior en pacientes mayores de 40 años(2017) Arbeloa Gutiérrez, L.; Arenas Miquélez, A.; Arenas Planelles, Antonio; Pablos Fernández, Julio deAim. Assess the results of the reconstruction of the anterior cruciate ligament (ACL) in patients over 40 years after a minimum follow-up of 24 months. Material and Methods. Retrospective review of 37 patients undergoing surgery for ACL reconstruction between 2002-2013, over 40 years with a minimum follow-up of 24 months. 25 patients were available for clinical review, which consisted of physical examination, radiographic and arthrometric KT-1000 studies, functional tests and subjective assessment of the knee through surveys (International Knee Documentation Committee (IKDC), Tegner and Lysholm-Tegner). Results. At follow-up, at least 2 of the 3 knee instability maneuvers (Pivot-shift, Lachman, Anterior drawer test) were positive in 6 of 25 patients (24%). The difference of antero-posterior laxity compared with the healthy knee using KT-1000 arthrometer to 89N was greater than 3mm in 5 patients (20%). None had a greater difference of 5mm. Only 6 patients recovered their previous level of activity, according to the Tegner test. However, the average score on the Lysholm test was 81,40 ± 19,56 points, with good or excellent results in 15 patients (60%). IKDC test results were less conspicuous, with good or excellent results in 7 patients (28%). Conclusions. The results after ACL reconstruction in patients older than 40 years old are good, with a high degree of satisfaction and are comparable to those in younger patients. Although return to previous activity level is less than in younger groups, age should not be a limiting to indicate an ACL reconstruction factor in an unstable knee.
- Necrosis avascular de cabeza femoral en pacientes con enfermedad de Gaucher tipo I : tratamiento mediante artroplastia total de cadera no cementada(2017) López Vega, Marcos; Blas Dobón, J.A.; Valero Fabado, H.; Pelayo de Tomás, José Manuel; Rodrigo Pérez, José LuisEl 80% de los pacientes con enfermedad de Gaucher presentan manifestaciones óseas, siendo la necrosis avascular de la cabeza femoral una de las que mayor limitación produce y la primera causa de intervención quirúrgica osteoarticular. La artroplastia total de cadera cementada ha sido el tratamiento de elección en los casos con mayor deterioro de la articulación, pero sus resultados a medio y largo plazo se han visto ensombrecidos por la elevada tasa de aflojamiento. La terapia enzimática sustitutiva ha disminuido las complicaciones postoperatorias, además de permitir el uso de componentes no cementados con resultados similares a las prótesis implantadas por otros motivos. Presentamos una serie de 5 artroplastias de cadera no cementadas, con un seguimiento medio de 12.4 años (3.4-30), en 4 pacientes con enfermedad de Gaucher tipo I y osteonecrosis de la cabeza femoral. Solo se observó un caso de aflojamiento aséptico a los 18 años de la intervención.
