- Situación global de las competencias y conocimientos en medicina músculo-esquelética en licenciados en Medicina. Revisión narrativa(Engloba Grupo de Comunicación, 2022) Roselló Añón, A.; Sangüesa Nebot, María José; Doménech Fernández, JulioMusculoskeletal disorders are one of the most frequent causes of disability, handicap and demand for medical attention. Knowledge of musculoskeletal medicine (MSM) should be essential for most specialties. Unfortunately, many medical interns (MIR) admit to a lack of confidence and competence in this area. The aim of this paper is to perform a narrative review of the level of knowledge and skills acquired in musculoskeletal pathology of medical graduates in different countries. The data show that the teaching of MSM is deficient during the academic period. It?s essential to plan training actions that lead to an increase in knowledge and skills in this pathology in medical students. Optimisation of pregraduate teaching in MME will help to improve healthcare, make the system more agile, and allow better screening of specialised referrals.
- Linfoma No-Hodgkin primario de sacro. Presentación de un caso clínico y revisión de la literatura(Engloba Grupo de Comunicación, 2022) Díez Albero, L.; Moril Peñalver, L.; Sevilla Monllor, A; Fornés Gallego, E; Lapeña Martínez, AIntroduction Bone lymphoma is a very rare entity. It constitutes less than 2% of lymphomas and the sacrum is an unusual primary location. Only 11 cases have been described to date. Objective To describe a case of primary sacral lymphoma in a 46-year-old patient after reporting lumbar pain with radiculopathy of one month's evolution. Material and methods A 46- year-old female patient with lumbar pain of one month's evolution of mechanical characteristics with radiculopathy S1 of the right leg. No acute bone injury was observed in the X-rays performed. A CT scan was performed in a private center that reported a lytic lesion of 28 x 97 mm diameter in the right sacral wing with malignant characteristics. MRI is requested, which confirms the lesion with extension to soft tissues. Tumor markers and plasma proteins were not elevated. Biopsy with histopathological analysis reported a diffuse large B-cell lymphoma with positivity for cMYC, BCL2, BLC6, CD20, CD79a, PAX5, MUM1, with a Ki67 index of 75%. Results After diagnosis of primary diffuse large B-cell lymphoma without distant metastasis, chemotherapy treatment was started with 6 cycles of R-CHOP followed by radiotherapy. Conclusion Primary bone lymphoma is a very rare entity, and even more rare at the bone level, but knowledge of this entity is necessary to include it as a differential diagnosis of low back pain with radiculopathy.
- Fracturas de cuello de radio Jeffrey Tipo II. Revisión de la literatura a propósito de un caso(Engloba Grupo de Comunicación, 2022) Martín Flor, I.; Fayos Jordán, J.L; Segura Serralta, J.D; Pardo Llopis, J.V.Introduction: The elbow is the most frequently dislocated joint in children, while radius neck fractures account for 1% of all pediatric fractures. In the literature, two types of fractures have been described in this location associated with elbow dislocation in which the head is completely displaced with respect to the neck, both types being infrequent. One of them was described by Jeffrey, who postulated that the displacement and fracture were generated from the spontaneous reduction of the transiently dislocated elbow. Case description: Following his description, we are going to present the case of a 9-year-old girl who suffered an isolated radial head fracture-dislocation. After closed reduction, a 180º inversion of the radial head was observed with respect to its metaphysis, requiring a subsequent open reduction and fixation with Kirschner wires. Discussion: Since Jeffrey described this fracture mechanism, multiple authors have published cases that reaffirm his hypothesis. In the same way, new maneuvers have been described that allow the correct reduction of this fracture while being the least bloody possible. Conclusion: Our purpose is to carry out a review of the literature regarding this case in order to highlight the importance of a correct initial assessment that Will lead us to the most appropriate treatment at all times.
- Fractura de estrés del cuello del fémur. A propósito de un caso(Engloba Grupo de Comunicación, 2022) Desviat Ruiz, M; Benavente Lazo, W.A fatigue fracture, also known as a stress fracture of the femoral neck, is an uncommon type of fracture that happens when a patient engages in strenuous physical activity or overuse. This case involves a 31-year-old woman who consults the hospital on several occasions with creeping pain in her groin and inner thigh. Following normal X-rays and the absence of clinical improvement, an MRI and a fresh X-ray were conducted, revealing a loss of continuity of the cortical bone of the femur's neck, which was classified as a stress fracture. The most common consequence of this type of fracture is femoral head necrosis due to delayed diagnosis. As a result, it's critical to get a diagnosis as soon as possible
- Esguince lateral de tobillo con necrosis cutánea no yatrogénica. Presentación de un caso y revisión de los sistemas de clasificación(Engloba Grupo de Comunicación, 2022) Miralles Muñoz, Francisco Antonio; Valero Bravo, M.N.El esguince lateral de tobillo es una lesión frecuente de rápida curación con tratamiento correcto. Presentamos un caso con complicaciones poco comunes, no relacionadas con tratamiento ortopédico inadecuado. Además se completa el estudio realizando una revisión de la literatura con los diferentes sistemas utilizados para clasificar este tipo de lesiones. Varón deportista de 49 años con esguince lateral de tobillo mientras practicaba baloncesto que presentó edema importante, que evolucionó a necrosis cutánea anterolateral en 2 semanas. Tras escarectomía, presentó herida profunda con exposición de estructuras cápsulo-ligamentosas, convirtiendo la lesión en abierta. Asoció lesión completa de nervio peroneo superficial. Se iniciaron curas ambulatorias con terapia de presión negativa asociada, cicatrizando la herida a los 4 meses de evolución, retornando a la práctica deportiva a los 8 meses.
- Luxaciones puras de astrágalo: tipos, características específicas y tratamiento. A propósito de 3 casos(Engloba Grupo de Comunicación, 2022) Marco Diaz, L; Bertó Martí, X.; Fuentes Leal, S.; Garrido Ferrer, J.F; Hernández Ferrando, L.The luxation of the astragalus are severe but uncommon injuries. These luxations associates a great sort of complications, so they can be a challenge for the surgeon. Tipically are consequence of high energy impact in young people. Less frequently these injuries happend as a result of low energy impact in people with a higher laxity of their joints or malformations.They often associates talus or ankle fractures, tendon rupture, o neurovascular damage. These lesions are named pure?luxation of the astragalus when there is not bone fracture. This article has the aim of describe the three types of pure?luxation of the astragalus by three case reports
