    Luxación aislada de la articulación radiocubital distal : Un nuevo caso

    Se presenta 1 caso de luxación dorsal aislada de la articulación radiocubital distal en un paciente varón de 18 años que sufrió una caída sobre la mano izquierda en posición de pronación del antebrazo. Fue tratado mediante reducción cerrada bajo anestesia local y posterior inmovilización en un yeso braquioantcbraquial con el codo en 90° de flexión y el antebrazo en supinación durante 6 semanas. El resultado tras el tratamiento rehabilitador posterior a la retirada del yeso fue satisfactorio, sin dolor ni alteraciones estéticas ni funcionales en la extremidad.

    Pseudoartrosis del noveno arco costal : caso clínico

    Se presenta un caso inusual de pseudoartrosis de la novena costilla en una mujer de 22 años de edad. La pseudoartrosis de la primera costilla ha sido descrita con relativa frecuencia, pero en el resto de los arcos costales es desconocida. El diagnóstico diferencial se hizo en base a las características clínicas y radiológicas. La persistencia del dolor y la imagen radioló- gica de pseudoartrosis obligó en este caso a la costectomía segmentaria.

    Técnica de Imhäuser en el pie zambo : resultados a medio y largo plazo

    —We cheked 42 patients with club-feet (60 feet), all of them with follow-up over 5 years. All the patients received a mixed treatment: orthopaedic (manipulation and plastering) and surgical (Imhäuser technique modified). We evaluated morphological, functional and radiographic parameters. In the overall evaluation only one of the cases was considered bad result; 13 (21.6%) fair results, 6 with valgus flat feet, 3 with pronatus forefoot and 4 with adductus forefoot; 12 (20%) good results, with a minimal morphologic or functional alterations; 34 (56.6%) were excellents results. Conclutions: 1) Imhäuser technic had 77% of good or excellent results in this work; 2) the second and third Imhäuser surgical times can be summarized in only one: the external hemitrasplant of the tibial anterior tendon, and 3) clinics, morphologics and functional results are better than radiographics results.

    Fracturas diafisarias de húmero : enclavijamiento intramedular con agujas de Hackethal por vía epicondílea lateral

    This study was based on 58 patients with diaphyseal fractures of the humerus, treated by Hackethal fascicular intramedullary pinning inserted at the lateral humeral epicondyle. We analysed the indications for the operation using the modified Hackethal technique. We evaluated the final clinical and radiological results, obtaining 85% good results and 15% poor results. The main cause for these poor results was the appearance of no-union due to diastasis of the fracture focus after reduction and the presence of traction and rotational forces in the postoperative inmobilization methods.

    Coste energético en gonartrosis y prótesis total de rodilla

    By mean of calorimetric indirect method with spirometry of closed circle we carry out a physiologic study of the exercise in fifteen patients with arthritis of the knee, fifteen with total knee arthroplasty type Press-fit and fifteen with total knee arthroplasty type endomodel of Waldemar-Link. There was an influence of pain and gait in oxigen consume, being significantly lower in patients with arthritis of the knee without knee arthroplasty. The best clinic and energetic results were found in patients with knee arthroplasty type Waldemar-Link.

    Luxación acromioclavicular: resultados del tratamiento con cerclaje alámbrico sobre agujas de Kirschner

    A series of 44 out of 57 patients with Allman grade III acromioclavicular dislocation operated on from 1975 to 1992 by a modified Phemister technique was studied. Mean age was 38 years. Functional Imatani scale, radiological and aesthetic results, and own patients opinion were considered to evaluate this procedure. Average follow-up was 6 years (range: 2-17 years). Results were considered excellent in 89%, good in 9% and poor in 2%. Anatomical reduction was achieved in 75%. In 91% of cases no clavicular protrusion was observed. A total of 75% were satisfied or very satisfied with their result. Superficial infection and implant failure were found to be the main complications. This technique can be a good procedure when a surgical treatment is considered. Shoulder function must be restricted until the implants have been removed.

    Estudio clínico-radiológico de la osteosíntesis con clavo gamma en fracturas trocantéricas

    Two hundred twenty two fractures of the trochanteric area treated with the gamma standard nail were evaluated retrospectively. The clinical outcome was assessed keeping in mind the functional result and complications. The type of implant, stability impression, anomalies of the assembly and valgus-varus angle were analyzed in order to study their repercussions in the consolidation. This series is compared with others in the literature with the same implant (gamma nail) and with others using other systems of osteosintesys, like DHS and Ender's nails. We conclued that with a purified technique the gamma standard nail is an ideal system for osteosinthesys of unstable fractures of the trochanteric area.

    Reconstrucción ósea acetabular con cotilos no cementados y aporte de aloinjerto en la artroplastia total de cadera : estudio de 35 casos

    Thirty five revision hip arthroplasties performed in 29 patients with acetabular bone defects were reviewed. In this series we only include the acetabular reconstruction performed using non cemented acetabular implants. The mean age of the patients was 60 years (29-79), 19 were female and 10 male. We classified the acetabular bone defects according to D'Antonio, in segmentary, cavitary and combined. To repair these defects, we used criopreserved allograft, except one case in which a liofilized allograft was used. The results were evaluated clinically (modified Harris hip score) and radiographically, looking for acetabular migrations, radiolucent lines, periarticular ossifications and bone graft incorporation. We obtained 60% of excellent and good results. There was 40% of poor and fair results. The acetabular loosening rate was higher than in revision hip arthroplasty without acetabular bone defects.

    La prótesis de cadera con recubrimiento de vidrio Primeros resultados con 2 años de evolución

    This work analizes the first clinical and radiological results of a cementless hip prosthesis with glass coating at metaphyseal level (CRM prosthesis). A total of 80 arthroplasties implanted since may 1991 to 1994 were studied with a two year follow-up. The clinical assesment was done by the modified Harris score, being satisfactory in 90% of the patients. The radiological study demonstrated good fixation and orientation of the prosthetic compounds in 72 cases. The prosthetic model studied provides a security mechanism both for primary and secondary fixation of the femoral stem and acetabular cup.
