    Artritis séptica de rodilla por Streptococcus agalactiae: presentación de un caso y revisión de la literatura

    This report describes a case of knee septic arthritis in a 69-year-old diabetic patient. At the time of admission he was receiving local treatment for a chronic skin ulcer in the same extremity and inespecific oral antibiotherapy because of an urinary tract infection. Synovial fluid cultures showed a Streptococcus agalactiae (group B streptococcus). The infection had no response to specific parenteral antibiotherapy and repeated joint irrigations-aspirations, and therefore surgical desbridement and synovectomy was required. Despite of an intensive rehabilitation program, the affected knee developed ankylosis. In a review of literature, it is remarkable the association between group B streptococcal arthritis and chronic skin ulcers in diabetic patients and the potential severity of these infections.

    Luxación inveterada del codo en niños: a propósito de un caso

    La luxación no reducida del codo es rara y no suele verse en los países desarrollados, salvo en grupos sociales marginados y de forma excepcional. La luxación más frecuentemente es posterior. Entre las 3 semanas y 3 meses, el tratamiento cerrado suele fracasar puesto que la interposición de tejido de granulación y fibroso intraarticular y la retracción de los tejidos blandos, impiden una reducción incruenta y estable. Nosotros recomendamos para luxaciones inveteradas, vistas en este período de tiempo, el abordaje posterior con sección del tendón del músculo tríceps y la reconstrucción de estructuras blandas periarticulares. Presentamos el caso de un niño tratado mediante reducción quirúrgica y cuyo resultado final fue muy bueno.

    Osteoma osteoide de rodilla Dos casos de difícil diagnóstico

    We present two cases of osteoid osteoma affecting the knee, a very infrequent localization and therefore of difficult diagnosis. The scintigraphy was found to be of valuable assitance in revealing the presence of this lesion at the knee and CT-scan was likewise useful for its precision in localizing the tumor as a basis for deciding upon a viable surgical approach. MRI did not contribute some date. After surgical resection of the tumor, a complete relief of the symptoms was experienced in both patients.

    Luxación posterior bilateral de la articulación escapulohumeral

    A case of bilateral posterior dislocation of the scapulo-humeral joint ocurring in a 51-year-old men after a convulsive attack is reported. Inmediately after convulsions, the patient refered pain and funtional disability in both shoulders and bilateral scapulo-humeral dislocation was confirmed by conventional radiograhips and CT-scan. Treatment consisted of closed reduction and inmobilization of the upper extremity in abduction and external rotation with a thoraco-braquial plaster cast.

    Alteraciones fisarias tras radiación láser

    In view of the present importance of Laser Radiation, and due to the lack of references about its effects on the growth cartilage, we designed an experimental model in order to asses it. A total of 50 rats were used, establishing five groups of study at 10 animals each, depending on the dosis of radiation applied percutaneously on the distal femoral phisys. After the structural, histochemical and morphometric study, we observed a series of alterations indicating a direct celular damage at the physion. From the results, it is concluded that laser radiation should be on absolute contraindication during the phase of bone growth or in the inmature skeleton.

    Transfusión autóloga diferida en cirugía primaria de prótesis total de cadera

    The results of deferred autologous blood transfusion in 50 patients who underwent total hip replacement are analyzed. Patients donated blood in weekly bases, last donation 72 hours before surgery. A total of 100 units were obtained by this method and 77 of them were used, wich means 91% of the blood transfused to these patients. Eight patients were transfused with homologue blood. No adverse reactions could be observed either in relation with the autodonation of blood or the autotransfusion. One month after surgery, all patients showed hemoglobin and hematocrit whitin normal limits. Autologous transfusion is a simple and efficacious procedure of blood replacement in primary hip arthroplasty.

    Aloinjerto como material de relleno de defecto óseo metafisario en las fracturas de platillo tibial

    The purpose of this study was to assess the efficacy of allograft bone as filling agent of metaphyseal defects in tibial plateau fractures, trying to find an effective alternative to decrease the morbility (donor site complications: painful, infection, increased surgical time...), without affecting bone ingrowth and early motion. A total of 13 cases treated by anatomic open reduction, rigid internal fixation and filling with frozen allograft bone have been reviewed. When evaluating results according to Honkonen, excellent results were found in 7 cases, good results in 4 and bad results in 2. Allograft bone seems to be an effective alternative for repairing metaphyseal defects associated with tibial plateau fractures.
