- Fractura bipolar de clavícula(1994) Zamora Navas, P.; Collado Torres, Francisco; Torre Solís, F. de laA case of bipolar fracture affecting both ends of the clavicle is reported. The lesion consisted of a Neer type II fracture at the distal segment and a fracture close to the proximal joint with the sternum. Surgical treatment was carried out by open reduction and Kirschner wire fixation stabilizing both sides. At follow up, clinical and cosmetic results are satisfactories.
- Experiencia a medio plazo con prótesis total de rodilla semiconstreñida(1994) Sánchez Gimeno, M.; Cardona Vernet, J.M.; Gómez Ribelles, A.; Orán Espuys, J.; Zamora Rodríguez, J. M.; Rocha Solé, M.; Escudero Fernández, A.; Fernández Martínez, J.J.We have reviewed 25 total knee prosthesis (Endo-Model type, Waldemar-Link) performed in 23 patients with an age average of 69,5 years-old and a average follow-up of 32 months. The clinical assessment applied was the AAOS protocol including the parameters of: pain, ability to go up and down the stairs, to need of sticks, flexion and extension of the joint, before and after the operation. In the radiological study we have evaluated the correct alignment of the components in two planes A-P and L, and the periprosthetic lucencies. The results obtained have been satisfying in all the patients with a diminution of the pain and better function of the joint. No deep infection was observed and the flexion was better in prosthesis with anterior deviation of the femoral rod in the lateral plane.
- Bloqueo de la articulación metacarpofalángica por osteofito de la cabeza del metacarpiano(1994) Ferreres Claramunt, A.; Suso Vergara, Santiago; Arandes Renú, José María; Ramón Soler, RobertA 79 years old woman that presented her middle finger metacarpophalangeal joint blocked in flexion because and osteophite on the dorsal-ulnar aspect of the head of the metacarpal is presented. Diagnostic was stablished after radiological study. Resection of the osteophite allowed the ulnar collateral ligament come back to its normal excursion and complete extension of the joint was regained.
- La transfixión percutánea cúbito-carpiana en las fracturas inestables de la extremidad distal del radio(1994) Fraga García, H.; Pernas Puente, Juan Antonio; Sampedro Costas, J.J.Se expone una alternativa terapéutica para las fracturas inestables de la extremidad distal del radio, consistente en estabilizar la reducción mediante una aguja de Kirschner que atravesando oblícuamente la extremidad distal del cúbito se ancla en el carpo. Se precisan los detalles de la técnica que, en una valoración inicial sobre más de cien casos, aporta resultados constantemente satisfactorios. Se hace énfasis en la sencillez del procedimiento, que al propiciar una ejecución muy rápida, cobra ventaja frente a otras modalidades (fijador externo, osteosíntesis abierta, etc.) y especialmente ante traumatismos asociados del mismo miembro.
- Osteoporosis transitoria de la cadera: revisión de aspectos clínicos y terapéuticos a propósito de un caso(1994) Ruy- Diaz García, A.; González Herranz, J.; Ballesta Vicente, F.; Rodríguez Ferrol, P.A case of transient osteoporosis of the hips in a 17-year-old-woman is reported. Transient osteoporosis of the hip (TOH) is an uncommon but possibly underdiagnosed condition. The TOH affects previous healthy middle-age men, and women in the third trimester of pregnancy. Its etiology is still unclear. Pain in the hip area and limp are the main clinical signs. The MRI study shows low signal intensity on the Tl weighted images and limp signal intensity on the T2 weighted images in the femoral head and neck. It is generally accepted that the change in MRI is caused by an increase in fluid within the marrow cavities of the femoral head. Differential diagnosis of hip problems is required. The purpose of this report is to review the diagnostic and therapeutic aspects of TOH.
- Osteoartritis luética de rodilla(1994) Panisello Sebastiá, Juan José; Calvo Díaz, A.; Tabuenca, A.; del Valle, C.; Lorda, J. L.Se presenta el caso de un varón de 59 años, con antecedente de contacto sexual sospechoso entre 3 y 5 años antes, que desarrolló episodios de derrame articular indoloro en rodilla izquierda con una lesión osteítica destructiva en meseta tibial interna. Tras descartar otras patologías, el diagnóstico se efectuó por la intensa actividad treponéndca demostrada en la serología específica y la buena respuesta al tratamiento antibiótico. Los derrames desaparecieron, persistiendo únicamente la deformidad residual. No conseguimos identificar espiroquetas en el material obtenido por biopsia osteoarticular.
- Tratamiento quirúrgico en las cifosis congénitas: Revisión de 14 pacientes(1994) Tabuenca Dumortier, F. J.; Hevia Sierra, E.; González Herranz, P.; Deglané, R.; Sánchez Vera, M.Fourteen patients with congenital kyphosis treated surgically between 1979- 1989 were reviewed. All had a follow-up of 2 years or more, with an average follow-up of 7 years. The average age at surgery was 11 and the average kyphosis was 79°. Six cases had posterior fusion only and eigth had combined anterior and posterior fusion. The results showed an average correction of the curve at surgery of 18° with posterior arthrodesis and 20° with combined arthrodesis. There was thus an average loss of 10° and 8° respectively from the time of surgery in both types of treatment. Pseudoarthrosis by short fusion ocurred in one case. Other complications after surgery were 1 radiculopathy, one wound infection and four rod protusion (six patients). The factors that have influence in this results were analysed. A comparison from the results of treatment at other medical centers was also carried out.
- Respuesta ósea al implante del compuesto beta-fosfato tricálcico-colágeno (ß-ftc-c). Estudio «in vivo» en conejos(1994) Meseguer Olmo, Luis R.; Alcaraz Baños, Miguel; Vicente Ortega, Vicente; Clavel-Sáinz Nolla, M.; Galián, A.Se ha estudiado la correlación entre los hallazgos histológicos y las imágenes radiológicas de un compuesto de beta-fosfato tricálcico y colágeno (ß-FTC-C) después de su implantación en defectos óseos creados experimentalmente en 21 conejos de 4 Kg de peso medio. Bajo anestesia general los defectos óseos creados en ambos fémures se rellenaron con un cilindro de 4,5 x 6 mm del compuesto. Los animales fueron sacrificados a la semana y 1, 2, 4 y 12 meses del postoperatorio. Las muestras obtenidas fueron sometidas a estudio radiográfico e histológico. Microscópicamente se comprobó que el compuesto a lo largo del período de implantación sufrió un proceso de reabsorción progresiva mediado por células de estirpe macrofágica y que se correspondió con una disminución de la densidad radiológica y cambios en su morfología. A los doce meses del postoperatorio se observaron escasos restos del material, hecho que se corresponde con la desaparición del implante en la placa radiográfica.
- Profilaxis de los puentes óseos fisarios mediante interposición de injerto autólogo de periostio. Estudio experimental en conejos(1994) Carpio, J.; Carranza Bencano, Andrés; Armas Padrón, J. R.; Saenz López de Rueda, F.An experimental work has been carried out on prevention of fiseal bone bridges formation by means of interposing autologus periosteum graft, bringing up the following objetives: 1) To achieve a barrier which can prevent the communication between epiphyseal and metaphyseal vessels, and 2) To study the condrogenic potencial of periosteum. A total of 48 rabbits have been used, arranged in three series of four groups each, acording to the moment of sacrifice in the 1st, 2nd, 4th and 8 t h weeks. They were all operated on the right femur. In the control serie an extirpation of 1/3 of the outer and distal growth plate was carried out, while in the experimental series the created defect was filled up with a free periosteum graft. All the specimens were studied macroscopically, radiologically and histologically. As conclusions, the periosteum used as a free graft: 1) In the first weeks act like a pasive barrier in physeal lesions, 2) It has also condrogenic capacity in the first weeks and, 3) On large follow-up studies, the periosteum do not seems to be a efective material to prevent physeal bone bridges.
