    Tratamiento quirúrgico en las fracturas del radio distal tipo A3

    Se presentan 44 casos de fractura de radio distal tipo A3 tratadas en nuestro Servicio entre los años 1992 y 2004. Se utilizó como método de osteosíntesis una placa palmar en 7 casos, agujas de Kirschner percutáneas en 14 casos, fijadores externos en 20 y fijador externo + agujas en 3 casos. Los mejores resultados se obtuvieron en el grupo de casos tratados con placa palmar (100 % de resultados satisfactorios).

    Etiopatogenia de la osteonecrosis idiopática de cadera: situación actual

    Al margen de las numerosas hipótesis etiopatogénicas sobre la osteonecrosis atraumática de cadera, la fisiopatología de la enfermedad sigue siendo, en la actualidad, motivo de debate. En los últimos años, han aparecido estudios que relacionan la aparición de la enfermedad con alteraciones subyacentes en la hemostasia de estos pacientes. Gracias al desarrollo de las técnicas de secuenciación genética, también se ha podido asociar la enfermedad con distintos polimorfismos genéticos de enzimas implicados en la fisiopatología de la osteonecrosis. En el presente artículo se ha realizado una revisión bibliográfica exhaustiva sobre los factores etiológicos ya conocidos como los corticoides, el alcohol y el tabaco, así como los de reciente aparición.

    Hiperplasia angiolinfoide con eosinofilia: a propósito de una localización atípica en músculo trapecio

    La hiperplasia angiolinfoide con eosinofilia es una patología poco frecuente de presentación clínica inespecífica que lleva a que rara vez se diagnostique antes del estudio histológico. Habitualmente afecta a piel y tejido celular subcutáneo siendo rara la localización extracutánea. Presentamos un caso clínico de una tumoración en una mujer de 46 años localizada a nivel del músculo trapecio; y revisamos las características histopatológicas y de tratamiento de esta patología.

    Método de Ponseti en el tratamiento del pie equino-varo-adductus asociado a hemimelia peronea tipo IA

    Fibular hemimelia is a congenital deficiency of the fibula associated with other anomalies of the lower limb of difficult treatment. There may be a shortening of the affected limb, and deformities and abnormal positions of the foot. Usually it is associated with equinovalgus deformity of the foot, but there are also cases of clubfoot, as in the case of the child presented in this report. Ponseti´s method was used to correct this deformity, and plantigrade foot was achieved. Although the results are not as good as in idiopathic cases, Ponseti´s method provides many advantages, as a partial correction is achieved, and this will allow a more limited posterior surgery.

    Rotura del tendón distal del bíceps braquial

    Objective: to present our experience in the treatment of ruptures of the distal biceps braquialis tendon in hand workers. Methods: 9 patients, middle age 47, went under surgery, 8 of them with a transoseus reinsertion of the tendon in the bicipital tuberosity of the radius with a double surgical approach. The average follow-up was 13 months. Results: excellent results were achieved in all of the 9 patients, recovering previous muscle strength, with full range of flexion-extension, 85º of supination and 86º of pronation. All 9 patients return to work in a period of 3 to 4 months. Conclusion: this technique is effective in the treatment of this injury, and it is recommended in active patients.

    Fractura de trasfondo acetabular en un niño de 14 años con defecto de cierre del cartílago trirradiado: seguimiento de 13 años

    Acetabular fractures represent between 1 and 15% of pelvic fractures in children. The early closure of the triradiate cartilage is one of its complications that can lead to a secondary acetabular dysplasia, so the chances are higher the lower the child's age at the time of the injury. The case of a child of 14 years old who suffer a traffic accident is described. He presents, among other injuries, a left acetabular background fracture, with non surgical treatment. After 13 years of follow-up, there is a persistent defect of closure of the triradiate cartilage.

    Fibromatosis extraabdominal agresiva: descripción de un caso y revisión de la literatura

    Desmoid tumour is a rare benign neoplasm of soft tissues, with local aggressivity and without metastatic possibility. There are different types depending on its location, the one we are going to review being the extraabdominal one or aggressive extra-abdominal fibromatosis, the one we are going to review, more frequent in scapular and pelvic waists. We report a case of a 35 years old man with a tumour in his left hip, with clinical findings of snapping hip. A review of the existing literature on this pathology is also carried out, with special emphasis on the biological characteristics of the tumour, as well as its histology, natural history and treatment.

    Toracoplastia expansiva en el tratamiento de la escoliosis congénita torácica con malformaciones vertebrales asociadas a fusión costal

    This paper reports on a 3-year-old patient with congenital thoracic scoliosis in the context of a VACTERL's syndrome, who had many spinal segmental defects with ribs fusion associated, in which an expansion thoracoplasty was made by means of a vertical expandable prosthetic titanium rib. An indirect correction of the scoliosis was achieved allowing the thoracic and respiratory development in the concavity of the curve and, thus, postposing the instrumentation with spinal fusion.
