    The orthopaedic surgery have today a position of change, not only at technical, professional, informative change, but also educational and trainig level. The education of future doctors and specialists is not satisfactory because of the presence of black points that must be eliminated or change through best educational situation at the medicine school and hospitals. The resident at present time remains a lot of hours working at the hospital, without best rest and formation; this situation must be reduced, for best education and training. The competences are offered like are introduced in America (USA, CANADA), intending improvement, as well the continous medical education up to have recertification. Finally, the author deplore the lack of investigation among residents; also, the characteristic of leadership are analyzed, always under the foundation of better attention to the patient and pay attention to the problems of our society

    Osteocondritis de los sesamoideos del hallux: revisión de la literatura a propósito de un caso

    Osteochondritis of the sesamoid bones is an unfrequent condition wich may affect both the medial and lateral hallux sesamoids. We report a case of osteochondritis of the external sesamoid in a 27 years old woman, with a one year clinical pain evolution, wich was satisfactory solved by carrying out its surgical excision. We compare the treatment carried out with others that have been published, reviewing the literature that refers to its causes, the diagnostic methods and treatment used
