- Hemotórax tardío tras fractura costal(2006) Carda Casalta, C.A case of delayed hemothorax in blunt thoracic trauma is presented. A man 45 years old with closed fractures of the ninth rib developed a hemothorax 21 days after thora- cic trauma.

There are different allogenic blood saving techniques but predeposit auto- transfusion is the gold-standard at present. We performed a retrospective study with the objective of valuing the requie- rement decreasement of allogenic transfusion in hip pro- gram orthopaedic surgery through blood saving methods, specially predeposit autologous transfusion. Autologous transfusion's great problem was the raised cost-effective- ness. The main reason for the price increase of techniques used is the high number of donned units and not transfused after surgery which you end up with a low productive met- hod. Anaemia pre-surgery and overtransfusion can easily be avoided with clear inclusion lines in blood saving programs and no liberal transfusion protocols. The best way to save allogenic blood in total hip arthroplasty can be established by means of an interdisciplinar program but individualized for specific tipe of surgery and for patient's characteristics like initial haemoglobin and sex. Predeposit autologous transfusion is adequate in hip program orthopaedic surgery because its effectiveness in allogenic blood saving, its good tolerance and it is a safe method.
- Aplasia parcial congénita de arco posterior de atlas.(2006) Varela Egocheaga, José Ramón; Montero Díaz, M.; Suárez Suárez, Miguel Ángel; García González, P.; Murcia Mazón, AntonioPartial aplasia of posterior arch of the atlas are rare but well documented anomalies, most cases are asymp- tomatic but cases of myelopaty and quadraparesis associa- ted to defects of posterior arch of the atlas have been publis- hed in the past. Case 1: female, 31 years old, suffered traffic accident with neck and upper limb pain. Partial aplasia of posterior arch to the atlas was detected in X-Rays, at the moment the patient continues with neck pain. Case 2: female 28 years old who referred neck pain after traffic accident, partial aplasia of posterior arch of the atlas was detected. Bibliographic review.

But the doctor must always support the patient et- hically and morally, while being immerse in the estate or pri- vate health service, which provide the necessary funding. “To be or not to be” is the current situation of the doctor, with the particularities of the orthopaedic surgeon. This work analyses the complexity of the doctor-patient relationship and the way of assist and sustain it as a founding stone of our profession, not forgetting the exceptional external circumstances that try to manipulate it. Moreover, professionalism is presented as the relationship of the doctor with society, with other doctors and with him or her self. Finally, there is special emphasis on the difficulty in obtaining reliable information for best practise in dealing with our patients as orthopaedic surgeons.
