El propósito de esta tesis es mostrar evidencias de la efectividad del tratamiento de Terapia Breve, con base a la hipnosis como coadyuvante (TBH), en el proceso psicoterapéutico de mujeres violentadas por parte de su pareja, o expareja. El problema de la violencia contra la mujer por la pareja ha sido ratificado como un problema de salud pública por la Organización Panamericana de la Salud (OPS, 2003a, 2010), tanto por el alto número de muertes de mujeres en manos de sus parejas, como por la morbilidad que en ellas genera el constante maltrato: sentimientos de vergüenza y culpabilidad, fobias, baja autoestima, Trastorno de Estrés Postraumático (TEPT), trastornos psicosomáticos, pensamiento e intento suicida, etc. Objetivo: Evaluar la efectividad de la Terapia Breve con Hipnosis (TBH) en la evolución personal de mujeres que sufrieron o estaban sufriendo, de violencia ejercida por la pareja al inicio del tratamiento. Está basado en el modelo ecológico adoptado por la Organización Mundial de la Salud (OMS) (Heise, Ellsberg, y Gottemoeller, 1999), para explicar y atender esta problemática, que propone aumentar los factores protectores y disminuir los factores de riesgo. Se seleccionó la hipnosis como estrategia central de la intervención porque ha demostrado ser beneficiosa en la práctica clínica, sobre todo como coadyuvante, en el tratamiento de una amplia gama de conflictos psicológicos y problemas de salud, como el manejo del dolor y otras condiciones médicas, existiendo evidencia aceptable de su eficacia especialmente como coadyuvante (Mendoza y Capafons, 2009). Método: La muestra estuvo constituida por doce mujeres. Siete completaron todo el tratamiento, tres recibieron una parte, y otras dos, se consideraron casos control. Se realizaron dos análisis, estudio de caso, y estudio de caso múltiple. Se establecieron tres categorías de análisis: 1) Diagnóstico y síntomas, tales como: depresión, ansiedad, insomnio, TEPT, pensamiento o intento suicida, etc.; 2) Tipos de violencia: psicológica, física, económica y sexual; ambas categorías desagregadas en niveles según la gravedad de las mismas; y, 3) Logro de las seis metas terapéuticas: psicoeducación, autoestima, identificaciones de emociones y sensaciones, límites personales, toma de decisiones, y comunicación asertiva; para lo cual se estableció una escala según el nivel de alcance. Procedimiento: Tres fases: i) evaluación; ii) implementación del tratamiento de TBH, con el uso de hipnosis, el cuento, la metáfora y la connotación positiva. iii) evaluación y seguimiento. Resultados: Las siete mujeres que concluyeron el tratamiento redujeron considerablemente los síntomas, particularmente el nivel de depresión y ansiedad. El nivel de violencia también disminuyó o se eliminó completamente al momento del cierre, que se mantuvo en las seis que volvieron al seguimiento. Asimismo, alcanzaron las metas entre un nivel 3 (alto-fortalecido) y 4 (óptimo) al momento del cierre y la mayoría (4) lo mantuvo en el seguimiento. En las tres mujeres que no completaron el tratamiento los resultados fueron menos notorios y con recaídas. Y, las dos mujeres que no obtuvieron el tratamiento (casos control), una mostró ligeras mejorías, y la otra, no mejoró, sino que tuvo deterioro en su estado de ánimo. El resultado más significativo es que seis de las mujeres que concluyeron la TBH, que cumplían con los criterios del TEPT al inicio del tratamiento, se evaluaron ya sin este diagnóstico al momento del cierre y el seguimiento. Conclusiones: Se lograron los objetivos y se comprobó la hipótesis. Se encontraron efectos clínicamente significativos de la TBH a partir de los beneficios que obtuvieron las participantes en su desarrollo personal y su relación de pareja con el rompimiento del ciclo de la violencia, integrados en importantes logros personales y sociales.

The purpose of this thesis is to show evidence of the effectiveness of a Brief Therapy, based on hypnosis as adjunt (BTH), in the psychotherapeutic process of women battered by their partner, or ex-partner. The problem of violence against women by the partner has been confirmed as a public health problem by the Pan American Health Organization (PAHO, 2003a, 2010), both because of the high number of deaths of women in the hands of their partners, stress posttraumatic disorder (PTSD), psychosomatic disorders, suicidal thoughts and suicide attempts, and so on. In addition, there is a lack of self-esteem. Objective: To evaluate the effectiveness of BTH in the personal evolution of women who suffered or were suffering from violence by their partner at the beginning of treatment. It is based on the ecological model adopted by the World Health Organization (WHO) (Heise, Ellsberg, & Gottemoeller, 1999) to explain and address this problem, which proposes to increase protective factors and reduce risk factors. Hypnosis was selected as the central intervention strategy because it has been shown to be beneficial in clinical practice, especially as an coadjuvant, in the treatment of a wide range of psychological conflicts and health problems such as pain management and other medical conditions, and there is acceptable evidence of its effectiveness (Mendoza & Capafons, 2009). Method: The sample consisted of twelve women. Seven completed the entire treatment, three received one part, and two others were considered control cases. Two analyzes, case study, and multiple case study were performed. Three categories of analysis were established: 1) Diagnosis and symptoms, such as: depression, anxiety, insomnia, PTSD, suicidal thoughts or attempts, etc.; 2) Types of violence: psychological, physical, economic and sexual; both categories disaggregated into levels according to their severity; and 3) Achievement of the six therapeutic goals: psychoeducation, self-esteem, identification of emotions and feelings, personal limits, decision making, and assertive communication; for which a scale was established according to the level of scope. Procedure: Three phases: i) evaluation; ii) implementation of BTH treatment, which included hypnosis, storytelling, metaphor, and positive connotation. Goals: psychoeducation, self-esteem, identification of emotions and feelings, personal limits, decision making, and assertive communication; (iii) evaluation and monitoring. Results: The seven women who completed the treatment significantly reduced the symptoms, particularly the level of depression and anxiety. The level of violence also decreased or was eliminated completely at the time of closure, which was maintained in the six that returned to follow up. Similarly, they reached the goals between a level 3 (high-strengthened) and 4 (optimal) at the time of closure and the majority (4) maintained it at the follow-up. In the three women who did not complete the treatment the results were less noticeable and with relapses. And, the two women who did not get the treatment (control cases), one showed slight improvements, and the other, did not improve at all, but had deterioration in their state of mind. The most significant result is that six of the women who completed BTH, who met PTSD criteria at the start of treatment, were already evaluated without this diagnosis at the time of closure and follow-up. Conclusions: The objectives were achieved and the hypothesis was verified. Clinically significant effects of BTH were found based on the benefits obtained by the participants in their personal development and their relationship with the breakdown of the cycle of violence, integrated into important personal and social achievements.
